Behavioral‑health staff present Q2/Q3 monitoring reports; Wellpath assumed jail clinical duties May 1

Behavioral Health staff presented a suite of Q2 and Q3 monitoring reports to the Sempervirens Joint Conference Committee at its June 3, 2026 remote meeting, and highlighted an operational change affecting future reporting.

Reports submitted and presented included seclusion and restraints (Q3), infection control (Q3), medical records, quality‑improvement tracking for May 2026, SV documentation monitoring, nurses/physicians/social‑worker reviews, fire and disaster drills, life and safety monitoring, SV staff training completion, morbidity & mortality (M&M) reviews, acute transfers, peer review for psychiatric prescribers, patient satisfaction, medication reconciliation, incident and unusual‑occurrence reporting, SV indicator dashboards, utilization review and the SV‑UR dashboard, and pharmacy and therapeutics monitoring.

Staff noted that, "as of May 1st, Wellpath took over clinical duties at the jail; therefore, future M&M reports will not include jail incidents." That operational shift was recorded in the meeting minutes and affects how M&M data will be compiled going forward.

The committee also handled routine administrative items: a correction to Jessica Duke’s title (remove “interim”), a minor typographical correction to report “H,” and a note that Cyanne Brocious would present the MCE report. No members of the public provided oral public comment during the session; written public‑comment instructions and a three‑minute limit for callers were noted in the meeting materials.
