Committee hears PCMH program update and schedules PCMH Plus data presentation for July

CHN told the Care Management Committee that as of May 31 the PCMH program included 124 practices, 553 sites and that member attribution stood at 54.6% of the HUSKY population; members asked for PCMH Plus Year 5 quality and provider-level data at the July meeting and flagged ER rates and CHW reimbursement for future discussion.
CHN provided the committee with the state's Patient-Centered Medical Home (PCMH) program metrics and a summary of quality-improvement engagement on June 10.
"As of May 31, we continue to have a 124 practices, 553 sites," the CHN presenter said, and reported that member attribution to PCMH providers was "54.6% of the total Husky population of 913,235." CHN also described engagement rates for quality-improvement activities (for example, 83% engagement for a PCA cohort) and preliminary measurement-year 2025 improvements overall.
Committee members asked for more granular PCMH Plus data. Members requested provider-level or provider-type breakdowns and emphasized emergency-department utilization as a gate to savings. Mercer and CHN representatives said they would try to present Year 5 PCMH Plus quality and shared-savings results ahead of the committee's July meeting; the committee set a July 8, 2026 meeting date.
The committee also identified fall agenda priorities: community health worker payment methodology, peer support services, and an update on the inmate medical program with the Department of Correction. Members volunteered to share CHW reimbursement documents and methods with DSS and staff.
Next steps: Mercer/CHN to pursue a July presentation of PCMH Plus results (year 5) if provider-session meetings can be completed beforehand; committee to consider CHW payment and peer-support topics in the fall.
