Clear Creek commissioners approve Collaborative Management Program MOU; county expects about $80,000 in incentive funding

The Clear Creek County Board of County Commissioners voted June 16 to approve a memorandum of understanding formalizing the county’s participation in the Collaborative Management Program (CMP), a cross‑agency family‑support initiative.
Sarah Casano of Human Services told commissioners CMP began in the state in 2004 to coordinate services for families involved with multiple systems (for example juvenile justice and human services) and that Clear Creek joined CMP in 2022 and began receiving incentive funding in 2023. “This year we anticipate receiving again another about 80,000 in incentive funding,” Casano said.
Casano described CMP as a forum to reduce duplication of services, braid supports across agencies, and hold family‑engagement meetings that identify needed services. Commissioners asked how referrals are made; Casano said referrals come from Human Services, schools, the courts, CASA and law‑enforcement partners and that families can self‑refer.
Commissioners praised the program’s prevention work and asked staff to continue outreach to increase referrals and report back on program use and outcomes. A motion to approve the CMP MOU was made and seconded; the board approved the item on the consent/action calendar.
The board also clarified a title correction for the agenda packet and agreed the full program overview should be provided to commissioners and staff as CMP referral activity ramps up.
