UNC Health strategic affiliation approved for Onslow Memorial Hospital

The Onslow County Board of Commissioners on June 15 approved a resolution authorizing a phased strategic affiliation between Onslow Memorial Hospital and the University of North Carolina Health Care System.
County counsel read a multi-page resolution describing a three-stage integration. Stage one — effective July 1, 2026 — grants UNC Health a 30% membership interest in Onslow Memorial Hospital and an expanded operational role; stage two increases UNC Health's interest to 49%; stage three, targeted for 2029, would make UNC Health the hospital's sole member. Presenters said UNC Health has committed to total investments of more than $150 million to support the integration.
Planning and legal provisions presented to the board include community-governance protections, county enforcement rights and lease amendments. Presenters emphasized that at all times at least 40% of OMH’s board of directors must be residents of the hospital’s service area or other local stakeholders, and that the county retains enforcement rights and biennial reporting and notice-and-cure protections intended to preserve local accountability. The resolution also contemplates a continuation and potential 65-year renewal of the county lease for hospital campus use.
Mr. DeSelms (presenter) summarized the documents before the board, which include county enforcement-rights language, amended and restated articles of incorporation and bylaws, and two lease amendments. County officials recognized representatives from Onslow Memorial Hospital who assisted with negotiation and noted extensive interagency and legal review. The board voted by voice to approve the resolution; no roll-call tally was recorded in the public transcript.
Supporters on the board described the affiliation as a historic step to sustain hospital services, bolster clinical resources and strengthen physician recruitment and retention. Commissioners said the affiliation is intended to preserve charity-care commitments and community responsiveness while improving the hospital’s operational stability.
The resolution takes effect immediately upon adoption. The county and hospital will complete the ancillary documents and related instruments required to implement the transaction, with non-material changes allowed to be approved by the chair in consultation with the county attorney.
What’s next: the county will execute the county enforcement-rights agreement and the second amendment to the lease and interlocal agreement on the chair’s authority, and parties will proceed with the staged integration described in the transaction documents.
