Operations staff reported to the commission that one February outage was driven by a breaker lockout caused by a flaw in breaker design that can cause jamming if a prescribed maintenance routine is not followed. Staff described the event as the principal driver of the month’s minutes-of-outage metric and said the utility is instituting a maintenance schedule and additional switching procedures to reduce recurrence.
The presenter said investigators initially suspected relay errors but determined those were display artifacts and that the root issue is a component/maintenance interaction in the breaker design. Staff said some breakers of the same make at multiple substations shared the issue, and that when breakers fail in this way they can affect a large portion of the system — a breaker outage can impact hundreds to more than a thousand customers, compared with a transformer outage that typically affects 4–10 homes.
To improve customer communications, staff described planned enhancements tied to the automated metering infrastructure (AMI) and a new customer information system: meters will report on/off status every five minutes and the utility will publish an outage map with estimated restoration times accessible on mobile devices. Staff also said the utility fixed call-routing gaps in its Doppler system and is rolling out opt-in text notifications to customers.
Commissioners asked for substation locations; staff identified the Gateway substation and others as involved and said the utility will refine maintenance and operational protocols to avoid long restorations in future outages.
