Commission approves one-year $175,000 pilot to subsidize GLP‑1 medications for employees after debate

Commissioners debated whether the county should subsidize GLP-1 medications and related clinician-supervised services after the insurance committee recommended a program that would provide up to a $200 subsidy per dosing event for participating employees. Concerns included potential clinical side effects cited by one commissioner (gallbladder disease, pancreatitis, gastrointestinal hospitalization risk), whether the drug is approved for the indications discussed, circumvention of the county’s pharmacy benefit manager, and an open-ended fiscal exposure if many employees enrolled.

Supporters said the program could improve employee health, retention and long-term savings and noted the county might structure the program to require participation in monitoring and coordination with PeopleOne or private providers. Commissioners discussed program design changes including caps and outcome monitoring; staff said program logistics and clinical-monitoring data would depend on provider arrangements and HIPAA constraints.

The sponsor amended the motion to run a one-year pilot beginning July 1 with a $175,000 cap for county contributions; the amended motion passed 4–1. The board directed the insurance committee and staff to develop metrics and reporting to the board so the pilot’s cost and health impacts can be evaluated before any extension or expansion.
