Mansfield House of Pizza cited for repeated food-safety violations; risk level raised to 3

Owner Keroles Mikhaeal told the board there had been “a miscommunication between the previous owner, and him regarding pumping services,” and that Baker Commodities is now contracted “to service the grease trap monthly.” Board members recounted a string of follow-up inspections by the regional health inspector, with violations found in September, October and again in December.
Board member Amy Donovan-Palmer told the board the December re-inspection included “2 priority violations, 4 priority foundation violations, and 4 core violations,” and described the number of violations as concerning. As a result, the board said Mansfield House of Pizza will be moved to risk level 3 and will receive three inspections per year.
The minutes record the owner’s statement that additional help has been added in the food establishment; they do not record a formal roll-call vote on the risk-level change.
