Health Facilities Commission ratifies license actions and approves multiple civil monetary penalties

The Health Facilities Commission unanimously ratified several initial licensure applications and waivers, accepted a facility closure notice, extended an inactive status through Aug. 2026 for a 419-bed nursing home, and approved civil monetary penalties totaling up to $12,500 across four providers.
The Health Facilities Commission on its regular agenda unanimously approved a package of license actions and disciplinary orders, including ratification of initial qualifying applications, a waiver under Regulation 0720-18-.04(1), a license closure notice and multiple civil monetary penalties.
The Commission approved an inactive-status extension for Nashville Community Care and Rehabilitation Center at Bordeaux (419 beds) for two years, through August 2026. A motion to grant the extension was moved and seconded and carried unanimously.
On ratifications, the Commission approved initial qualifying applications for several Home Health agencies, including America's Home Health LLC, Coal Miner Nursing Services and Perry County Home Health, and ratified multiple home medical equipment providers. Motions to ratify these initial applications were presented and adopted without objection.
The Office of Legal Services asked the Commission to assess civil monetary penalties against four providers. The Commission approved the recommended assessments as moved by Commissioner James Wright and seconded by Commissioner Melanie Keller: - Community Care of Rutherford County, Inc., S.N.F. ' one Type B civil monetary penalty of $500. - The Lantern at Morning Pointe of Franklin, A.C.L.F. ' four penalties of $500 and two penalties of $1,000 (total $4,000). - Brookdale Goodlettsville, A.C.L.F. ' one penalty of $2,000. - Gardens of Germantown, A.C.L.F. ' two penalties of $1,500 and one of $3,000 (total $6,000).
The Commission also accepted notice that the Center for Oral Facial and Implant Surgery (ASTC #104) will close its license effective Sept. 24, 2024. All motions on these items were approved unanimously.
The Commission recorded the routine motions and unanimous votes in the meeting minutes; the decisions will be reflected in the Commission's formal orders and licensing records.
