Commission ratifies multiple disciplinary fines and issues summary suspension to Candlelite One

The Tennessee Health Facilities Commission on June 24 ratified a slate of disciplinary consent orders assessing civil monetary penalties against several assisted‑care and staffing providers and issued a summary suspension to Candlelite One (RHA #519) after the facility failed to appear as requested.
The Office of Legal Services presented proposed consent orders with the following penalties: Vitality Living Hendersonville (ACLF No. 321) — three civil monetary penalties of $500 each (total $1,500); Southbreeze Assisted Living (ACLF No. 363) — one $1,000 and two $500 penalties (total $2,000); Stones River Manor, Inc. (ACLF No. 142) — probation up to 12 months plus a $3,000 civil monetary penalty; The Village at Bellevue (ACLF No. 196) — two $2,000 and one $500 penalties (total $4,500); Maristone of Franklin (ACLF No. 307) — one $1,000 penalty.
Temporary staffing registries — American Medical Staffing (TSA No. 84), KARE Technologies, LLC (TSA No. 170), and Advantis Medical Staffing, LLC (TSA No. 158) — each faced proposed consent orders assessing $100 civil monetary penalties plus costs not to exceed $1,000 and requirements to submit financial data within 30 days if not previously provided. Dr. Andy Russell moved to ratify the TSA orders and Mr. Mike Ussery seconded; the motions were approved unanimously.
Separately, Candlelite One (RHA No. 519) did not appear for a requested appearance by its administrator. After discussion, Commission member James Wright moved to issue a Summary Suspension order; Michael Denney seconded the motion, which carried unanimously.
The meeting minutes record the amounts, probation conditions and reporting requirements tied to the consent orders. Affected licensees are required to comply with payment and reporting timelines specified in the consent orders or appear before the Commission as directed.
