KDADS proposes single person-centered service plan for CSW; details TCM, MCO and CDDO roles

KDADS presenters walked stakeholders through proposed person-centered service-planning changes for the Community Supports Waiver and clarified the roles of TCMs, MCO care coordinators, CDDOs and FMS providers.
Elian Sorensen said Appendix D will describe a single person-centered service plan that replaces separate person-centered support and service plans to increase consistency and reduce burden on participants. Intake and eligibility begin with the Medicaid Functional Eligibility Instrument (MEFE) conducted by the CDDO; KDHE and KDADS confirm eligibility and approve waiver enrollment. Once enrolled, the MCO completes a needs assessment using the interRAI tool and the TCM develops the service plan using that assessment and other documentation.
Sorensen outlined key responsibilities: the TCM organizes and helps lead service-plan meetings, invites providers and supports participant choice; the TCM provides participants’ chosen providers with copies of the plan, develops backups and behavior-support plans (including convening a behavior-management/human-rights committee when necessary), and conducts at least quarterly face-to-face monitoring. MCO care coordinators must ensure plans meet requirements and that required forms and signatures are present before authorizing services. CDDOs are responsible for options counseling, monitoring provider compliance, conducting quality reviews when concerns arise, and uploading eligibility documents to the case system.
On self-directed services, Jackie explained the FMS provider will employ support brokers who work one-on-one with self-directing participants; the FMS handles administrative functions like payroll and tax withholding while support brokers provide training and hiring support. Michelle said the RFP for FMS support services is in procurement and expected to post in about 30 days.
Speakers emphasized these are draft processes for the CSW; KDADS said some IDD-waiver alignment and longer-term changes will be addressed later as the agency prepares renewal timelines.
