A Yamhill County presenter told the council that FamilyCore began as a simple referral exchange and evolved into a countywide system with a single referral form, memoranda of understanding, business associate agreements for HIPAA-protected sharing, and dedicated staff to coordinate referrals and professional development.
"The primary components of our FamilyCore is a shared referral. Everybody in the community uses the same referral form," the presenter said, noting that the county now has a full-time staff position dedicated to FamilyCore work and periodic regional trainings for home visitors. The system also includes weekly coordinator meetings to triage referrals and monthly directors' meetings to address strategic planning.
Panelists said embedding referral workflows into clinical site visits and existing collaborative meetings helped integrate home visiting without creating extra meetings for already-busy providers.
