Central Oregon officials say Family Connects improved provider coordination and outreach

Central Oregon panelist Paola described how the region implemented Family Connects and used relationship-building with medical providers and community partners to embed the program in existing workflows.
"Family Connects is a universal nurse based home visiting program for all families with newborn babies. There are no requirements to receive a visit," Paola said. She said the hub secured monthly notices in a provider association newsletter, daily patient lists from a regional clinic and continuing medical education presentations to raise awareness and recruitment.
Panelists added that initial concerns from older home visiting sites about referrals or "stealing" clients were addressed by inviting program leaders to regional case conferences so partners could clarify eligibility and referral pathways. Council members said these local outreach tactics could inform a statewide, model-agnostic awareness campaign.
