African American Health Program details SMILE, chronic‑disease and screening work and FY25 outputs

Waina Dixon, program manager for the African American Health Program, outlined AHP’s mission to eliminate health disparities and improve years and quality of life for African American residents and people of African descent in the county.
Dixon listed five core disparity areas the program targets (economic stability, access to care, chronic disease burden, maternal and infant health, and homeownership gaps) and described AHP’s approach: community education and engagement, prevention and risk management, clinical and care management and referrals to clinical providers. "Our overall mission is to eliminate health disparities and improve the number and quality of years of life for African American residents and people of African descent in the county," Dixon said.
She highlighted FY25 outputs the department attributes to AHP activities — screening numbers, more than 3,000 community referrals and a reported 6% average weight reduction for certain cohorts — and named program expansions (remote patient monitoring, PrEP outreach and increased screening efforts). Dixon also noted coordination with the Black Physicians' Health Network to improve referral pathways to primary and mental‑health care.
