"Families with children with a certified disability eligibility may happen through 1 of the following ways: Through the social security administration, the state medical review team or SMART. They may be qualified through MN choices assessment and or have a lead agency rule 1 85 developmental disability case manager," the presenter said.
The presenter clarified exclusions and program interactions: a child permanently residing in certain licensed residential settings (intermediate care facilities, nursing facilities, regional treatment centers, or other DHS- or Minnesota Department of Health–licensed facilities) is ineligible. The video also states a person cannot receive an FSG at the same time as waivered services (examples named), though families may close one program to enroll in another and may return later.
