Brian Brannon, chief financial officer for My Health Onsite, said the model is direct primary care for county employees (no CPT‑based billing) and reported about 88,239 completed appointments across the program and 7,572 medical visits used to compute per‑visit costs. Brannon showed utilization metrics (83.7% utilization in 2025 vs. a target of 85%), a high unique participation snapshot (94% of eligible employees counted in the December eligibility file), and outlined lab, Rx and wellness program activity. He presented an estimated net savings compared with the county plan—after staff costs and administration—of roughly $783,000 for the year and argued that the clinic reduces plan utilization and provides convenient same/next‑day access for staff.
Commissioner Rebecca Bays, citing rising claims and the county's self‑insured status, asked for a one‑year renewal and time to benchmark My Health Onsite numbers against Blue Cross Blue Shield claims. Commissioner Bays said, "I would like for us to take a deep breath, do a one‑year renewal, and really go in and analyze," and raised concern that the renewal had not been taken to bid since 2011. Legal counsel noted the current contract term is a five‑year term with a mutual renewal option and that the county can extend and then exercise a 90‑day cancellation for convenience.
After additional questions from commissioners and brief public comment asking for clarity on taxpayer costs, the board voted 4‑1 to renew the contract for five years, with the option to cancel on 90 days if staff needs time to pursue procurement or benchmarking.
Board direction: staff will provide additional cost breakdowns and benchmarking data to respond to commissioners' requests about claims trends and the clinic's net effect on the county's self‑insured plan.
