Tracy Rodriguez-Biggs of Providence Community Health Napa Valley presented a PowerPoint and a short video describing the PACE model. Rodriguez-Biggs said PACE — the Program of All-Inclusive Care for the Elderly — has been operating in Napa for three years and relies on an interdisciplinary team to coordinate medical, social and long-term-care supports for eligible older adults.
During the presentation she described eligibility criteria and how PACE is reimbursed, and she summarized who the program is intended to serve, referring to the "ideal candidate for PACE." Commissioners were invited to have the same presentation delivered to the Healthy Aging Planning Initiative (HAPI) later in the year.
