Anti‑obesity and GLP‑1 agents dominate State plan drug spending

Capital Rx reported that, for the full population in Q4 (Oct–Dec 2025), plan pay for pharmacy was $25,118,808 with patient pay of $1,413,986 across 73,675 claims (average gross cost per claim $360.13). By drug class, Anti‑Obesity Agents were the single largest class by plan spend at $4,892,189; the top five drugs by plan spend were Zepbound, Wegovy, Humira (2 Pen), Stelara and Mounjaro.
The report broke down utilization by population: active members accounted for plan pay of $22,573,748 (66,176 claims) and retirees for $2,545,061 (7,499 claims). Capital Rx’s clinical overview noted 922 unique GLP‑1 utilizers (2,088 claims) with plan pay of $2,600,967. Given the concentration of spend in a small number of high‑cost drugs and drug classes, the Commission’s Plan Design package includes a required engagement component for Virta Responsible Prescribing to manage GLP‑1 access.
Capital Rx also reported prior authorization activity for the plan: of 2,345 prior authorization cases, 1,703 were approved (73%) and 642 denied; average turnaround time was 23 hours. The Commission discussed these utilization trends in the context of plan finances and stop‑loss pressures.
