Nicole laid out the PSH prioritization: first priority goes to people exiting state hospitals or other state‑specified populations; second priority to chronically homeless individuals (HUD definition); third to literally homeless individuals with disabilities; and fourth to people in transitional settings at imminent risk of homelessness. She emphasized referrals must come through an active case manager at the county’s Department of Human Services and that a fixed nighttime residence generally means lower prioritization for PSH.
