Board plans final outreach after undeliverable disability affidavit; suspension possible if no response

Staff reported to trustees that disability-affidavit follow-up is incomplete for several members. One affidavit for William Daley was said to be en route and expected to be returned soon; a second mailing to David Ewell was returned undeliverable. Gregory Sweets is awaiting physician notes. Staff asked whether the board wanted another outreach attempt for the unresponsive member; trustees directed staff to attempt one more mailing and to try calling the member, with the possibility of issuing a final notice and scheduling a hearing to consider suspension of benefits if no response is received by the next meeting.
Staff said the original mailing went out on May 2 and the second on July 1; they recommended additional outreach prior to initiating benefit-suspension procedures. Trustees did not schedule a hearing immediately but authorized an additional attempt to contact the member and to escalate at the next meeting if there is no response.
