Senate passes House File 2635 to change prior-authorization, CON rules

The Senate moved and passed House File 2635, a bill the sponsor described as reforming prior authorization processes and parts of Iowa's certificate-of-need (CON) law. "Iowans deserve access to the best quality health care at the lowest possible cost," Senator Warmie said in opening remarks. He told colleagues the package includes reforms to CON (a law enacted in 1977 with thresholds last increased in 1997), changes to how insurers handle prior authorization and protections tied to peer review. Warmie said the bill aligns with the state's rural health transformation grant work and referenced a grant application for $1,000,000,000 and an award of $209,000,000 for the first year.
The Senate adopted two floor amendments before final passage. Senate amendment 50 60 was presented as a conforming amendment tied to House File 2635 and was adopted by voice vote after Senator Warmie moved its adoption. Senate amendment 50 67 made clarifying changes, addressed safety concerns about sharing personal information of clinical peer reviewers, added provisions on costs and timeliness for prior authorization requests and "requires all prior auths to be submitted electronically starting 07/01/2027," Warmie said. The amendment was also adopted by voice vote.
After the amendments, Senator Warmie moved that House File 2635, as amended, be read for the final time and placed on its passage. The secretary read the bill text on the floor; the roll-call vote recorded multiple named "aye" votes (including Webster, Zumba, Knox, Dawson and Evans) and the clerk announced "44" ayes and "0" nays. The chair declared the bill to have passed the Senate and the title was agreed to. Following passage, the Senate ordered House File 2635 immediately messaged.
The bill's backers argue it will reduce unnecessary regulation and administrative cost, expand quick access for services (particularly in cancer screening and certain behavioral-health settings excluded from CON), and maintain state oversight where community need and economic impact warrant it. Opposing or alternative viewpoints were not recorded on the floor during the final passage sequence.
