Cudahy embeds Community Health Worker in Quick Response Team, digital resources to expand

Health Officer Heather Puente reported that the Community Health Worker (CHW) hired for substance‑use and overdose response completed peer specialist training in April 2025 and is embedded in the Quick Response Team (QRT). Puente said the updated referral process was to be fully implemented in May 2025 and that the CHW had worked with 13 residents for about 19.3 hours to date.
Staff also told the board they had signed a contract with CredibleMind to expand digital resources on the South Shore CARES website, with an anticipated launch date in June or July 2025. Puente said staff will continue partner outreach to expand access to reproductive and behavioral health services through clinics and community agencies.
