Durham County keeps Cigna for self-funded health plan after RFP; pharmacy changes projected to save $2M

Human Resources presented the results of a competitive RFP that covered administrative services, pharmacy benefit managers and stop-loss insurance for the county'9s self-funded health plan.
Alex (benefits lead) told commissioners the county received proposals from Aetna, Blue Cross, Cigna, MedCost and UnitedHealthcare plus several PBMs and stop-loss bidders; staff recommended remaining with Cigna because a refreshed proposal improved pharmacy-cost guarantees and reduced member disruption. "We're projecting about $2,000,000 in cost reductions because of this new pharmacy contract," Alex said during the presentation.
Staff also detailed cost drivers: high-cost claimants and stop-loss increases, rising utilization, and new pressure from GLP‑1 weight-loss medications. To limit cost growth staff said the county will add utilization-management controls for GLP‑1 drugs beginning July 1 and increase outreach to use the county'9s on-site wellness clinic.
Next steps: HR will return with final contract documents for a July 1 coverage renewal and provide the board a fuller breakdown of first-year net savings once final vendor pricing is executed.
