Board approves wide range of disciplinary stipulations, fines and revocations

During its Oct. 15–16 meeting the Nevada State Board of Pharmacy approved a series of disciplinary Stipulations and Orders resolving allegations across several matters.
Examples recorded in the minutes include: Evergreen Pharmacy (License PH04174) was fined $5,000 and assessed $1,000 in attorney’s fees; Paula Lynne Santos (Certificate No. 20578) was fined $1,500 and assessed $500 in fees. Annette Asbury (CS25810) received a public reprimand, a $5,000 fine and $1,000 in attorney fees. Greenbay Specialty Pharmacy (PH04393) was issued a public reprimand, a $10,000 fine, $1,000 in fees and one year of probation. Stipulations were also approved in cases involving Walgreens locations and staff, with recorded fines and fees due by Dec. 1, 2025. The Board revoked Dr. Kristin Hestdalen’s Certificate No. CS38528 effective immediately under the terms recorded, with reinstatement allowed only after petition and a one‑year waiting period.
Board counsel prosecuted the matters and the Board approved the Stipulations and Orders, generally by motion and recorded action. The minutes show recusals where conflicts were declared (for example, Jade Jacobo recused from Item 4GG and from matters where employment created a conflict) and unanimous or recorded majority votes for approvals.
