During the Sept. 25 meeting, Susan Lyons raised that the "Non-Recoverable Cremains" form for Louise Longchamp (daughter of Clifford Hebert) had not been completed for cemetery records; Joe Mangan said the funeral home provided a check and a Certificate of Cremation but the signed form was missing. The commission emphasized the purpose of the form: to mark exactly where cremains are placed within a lot for its records.
Lyons said she will send the required paperwork directly to funeral homes in the future to avoid missing documentation. "One of the reasons that we ask for the Grave Opening Authorization form is to have it marked as to where the cremains/body is buried on the lot," she said. The commission noted the Hebert stone lists two names and clarified the placement is recorded in their records (for example, "2 feet in front of the name on the stone").
