CPMT approves streamlined FAPT IFSP; changes to take effect Aug. 1 if implemented

Hanover County’s Community Policy and Management Team voted June 25 to approve revisions to the Family Assessment and Planning Team (FAPT) Individualized Family Services Plan (IFSP) intended to streamline case manager workflow and FAPT decision-making.
Mr. Matthew (Matt) Eakin, the county CSA staff member who presented the revisions, said the changes were targeted rather than a complete revamp. The revisions remove duplicate "family vision and youth strengths" fields that replicated CANS scores, align goals with Department of Social Services needs and current placement, add a dropdown to indicate short‑term objective service status, expand foster care fields to capture transportation and daycare providers, limit the services section to clinical services with qualifier goals, consolidate the CSA‑Only section to be completed at the end of FAPT, and move utilization review questions to the FAPT table to improve efficiency.
"The proposed changes will go into effect on August 1, 2026 if approved by CPMT," Eakin said during the meeting. On a motion by Claiborne Warner, seconded by Gabriella Caldwell‑Miller, the CPMT voted unanimously to approve the proposed IFSP changes.
