Nevada Board of Pharmacy imposes fines, suspensions and revocations in multiple disciplinary cases

At its March 4–5 meeting the Nevada State Board of Pharmacy approved stipulations and orders imposing fines, probation, suspensions and revocations across multiple licensees and registrants, including revocation of a practitioner’s controlled-substance registration and a medical-products provider license.
The Nevada State Board of Pharmacy on March 4–5 approved a series of disciplinary Stipulations and Orders that imposed fines, probation and license revocations across multiple pharmacists, pharmacies and medical-product providers.
General Counsel Brett Kandt and other Board prosecutors presented cases including the amended stipulation for Ivan Goldsmith, which stayed a two-year suspension, placed Goldsmith on probation and imposed a $5,000 fine plus $1,000 in attorney fees. The Board approved the stipulation unanimously. The Board also revoked the pharmaceutical registrations of Susan Nytes for multiple disclosure and controlled-substance violations; her certificates were revoked and she was fined $5,000 with $1,500 in fees, and may not petition for reinstatement for two years.
Other actions included a stayed two‑year suspension and fines for Thomas Umbach (revocation of PD00512 with a five‑year reinstatement bar), and a Stipulation for Jill Allison Oliver that imposed a stayed one‑year suspension with a $2,000 fine and probation. The Board found Southwest DME guilty on Counts One through Four and revoked License No. MP03145, assessing a $5,000 fine and $1,240.92 in attorney fees. Most motions were passed by unanimous vote; several matters were continued to future meetings.
