The Policy/Personnel Committee reviewed Appendix C (instructions) and Appendix D (the complaint form) and directed staff to incorporate the form and instructions into administrative procedures. Appendix C currently instructs employees to "Submit the completed form to County Administration, attention Human Resources," and notes that confidentiality cannot be guaranteed. Committee members asked that procedures also specify a timeframe for completing an investigation and lay out what happens during an investigation.
Appendix D includes fields for complainant information, incident details, witnesses, and a county-use section for investigator assignment, findings and corrective actions. Committee direction was recorded as a recommendation to formalize where the form and instructions live in county procedures and to define the investigation timeline; the minutes reflect discussion and recommendations but not a formal recorded vote.
