Nevada Board issues fines, probation and suspensions in multiple pharmacy disciplinary cases

The Nevada State Board of Pharmacy approved a series of stipulated orders and disciplinary actions during its April 15–16, 2026 meeting, imposing fines, probation, suspensions and other conditions across multiple pharmacy licensees and registrants.
General Counsel Brett Kandt and staff presented factual allegations and proposed penalties in several matters. Renown Pharmacy and pharmacist Oliver Swafford were subject to stipulated penalties (Renown: $10,000 fine and $1,000 in attorney fees; Swafford: one-year probation, $2,000 fine, $500 in fees). The Board also approved fines and sanctions for CVS Pharmacy locations and supervising pharmacists, with amended penalties for CVS Pharmacy #9586 set at a $5,000 fine and $500 in fees. Michael Stellmacher’s amended stipulation increased the fine to $5,000 and added a five-year restriction barring him from serving as medical director at sites where injectable dangerous drugs are in possession of unlicensed individuals. Other actions included the retroactive revocation of Raymond Graber’s registration and suspension and probation orders for pharmacy owners who failed to notify the Board about asset sales.
Board motions to adopt the stipulated orders were routinely moved by Board members and passed as recorded in the minutes. Many orders include monetary penalties due by specified dates and continuing-education or probation conditions; in several cases licenses were placed on inactive status or suspended. The Board recorded these enforcement outcomes in public session and entered orders consistent with the accusations and stipulations presented by counsel.
