Davidson Countys Community Health team expands workshops, Triple P parenting and postpartum visits

Crystal Swink, maternal/child health supervisor, presented a program spotlight on the Community Health team and described an expanded, community-centered approach to outreach and services.
Swink said the team provides tailored health and chronic-disease education, wellness workshops, community case management inside the clinic, Triple P (Positive Parenting Program) parenting classes that support families involved with foster care and CPS, comprehensive car-seat checks by appointment, and postpartum/newborn home visits for Davidson County residents with active Medicaid. "We want to empower residents to make informed decisions about their health care and their overall health," Swink said.
The presentation listed partners and audiences served (senior centers, YMCA programs, recovery services and local schools) and noted the team also offers medication-administration training for community agencies that employ unlicensed assistive personnel. Swink supervises a public health planner and two community health social workers who deliver the services, and she also supervises the care management team.
