Quogue posts 2025 fire alarm permit form, fees, and workers' compensation requirement

The Village of Quogue included as Exhibit A a Fire Alarm Installation Permit Application form prepared under the Suffolk County Division of Fire Prevention. The form lists a New application fee of $200; amended plan submittal and re-inspection fees of $100 each; and a renewal fee of $100. The form states that the permit "expires 180 days after the date of issuance."

The form names David Schaffauer and gives a contact mailing address (PO Box 926, Quogue, NY 11959) and includes a telephone and fax number for the Suffolk County Division of Fire Prevention. It instructs applicants to submit proof of workers' compensation compliance with the application unless a certificate is already on file, citing Sections 57 and 220 of the New York State Workers' Compensation Law and Section 125 of the General Municipal Law. The application warns that "any false statement made herein is punishable as a misdemeanor, pursuant to Section 210.45 of the New York State Penal Law."

Applicants are asked to include a self-addressed, stamped envelope for return of plans and permits. The form also lists office-use fields for tax map number, payment receipt, workers' compensation expiration date, workflow status, permit number and approval/denial checkboxes.
