Pueblo County extends Resource Exchange subcontract two months as state program changes loom

Pueblo County approved a two-month extension to the independent contractor agreement with the Resource Exchange to continue providing long-term care home- and community-based case management services through April 30, 2025.
Rajon Ricotone (name as recited in the record), who presented the item on behalf of the department, said the county’s Single Entry Point program has subcontracted these services for more than 20 years. Ricotone cited three reasons for the short extension: the statewide roll-out of the Community First Choice program effective July 1, 2025; uncertainty about targeted case management rates (which Ricotone said account for roughly 73% of the program’s revenue); and the need to negotiate contract language specific to participant-directed services that are more resource intensive.
Ricotone reported recent revenue figures: for October through December the program received $567,237 versus anticipated $527,298, and that monthly averages rose from about $166,443 in the second quarter to roughly $189,000 in the third quarter. He said the two-month extension will give staff time to evaluate the impact of the state changes and to negotiate caseload and rate terms.
Commissioners approved the extension by voice vote.
