Payroll posting error on Dec. 26 traced to wrong file; county tightens controls

Scott Park, Davis County controller, briefed the Audit Committee on a limited review of a payroll posting problem that occurred on Dec. 26, 2025, a county holiday. Park said the wrong file was sent to the bank (accounts-payable file duplicated), the bank did not process it and, because of the holiday, the county could not resubmit the correct payroll file until the next business day.

Park said the underlying controls were designed properly but were not executed in this instance: email confirmations were forwarded and reviewed but staff did not notice that the control total in the submission did not match expectations. He described the event as a rare, coincident human-error sequence and said payroll was completed successfully afterward. "It was a human error... a blue moon very rare that a couple of things just happened to go wrong at the exact same time," Park said.

As remediation, Park said the treasurer's office and HR will double-check that control totals match before submission. The county also arranged same-day ACH processing when payroll files are submitted by the bank's cut-off (Park referenced an 11 a.m. submission window in discussion), which would permit corrective resubmission on the same morning in future similar cases. Committee members noted they had not heard of any employee hardship from the delay.

Park said the incident was a learning opportunity and that additional eyes on confirmation emails had already been added to reduce recurrence. The committee did not take formal action beyond requesting continued monitoring and a follow-up if additional changes are needed.
